Chronic Renal (Kidney) Failure
Renal care at the Center is focused on helping patients achieve maximum health and improved quality of life. NYU Winthrop’s Division of Nephrology and Hypertension, staffed by highly specialized physicians, nurses, dieticians and social workers, offers a comprehensive approach to the diagnosis, treatment and management of chronic renal disease. This includes a customized management care plan to ensure the best possible results. The Center also provides patient and family education, as well as access to the latest research advances and ongoing clinical trials.
Congestive Heart Failure (CHF)
The Center offers patients with heart failure access to treatment options, assistance with medication management and education on how to best manage this chronic condition. Through NYU Winthrop’s Heart and Vascular Services, patients are provided with a range of cardiac care services including access to the Heart Failure Center, where patients benefit from a multidisciplinary team that includes highly specialized physicians, nurse practitioners, dieticians and cardiac rehabilitation specialists who are highly skilled in assisting with the management of heart failure.
Patients with diabetes gain access to many services and treatment options through the Center for the Advanced Care of Chronic Conditions. Specialized healthcare professionals work closely with NYU Winthrop’s Specialty Care Program to provide comprehensive care for individuals with, or at risk for, diabetes. NYU Winthrop’s clinical Diabetes Program combines physician expertise with the expert support of nurses, nutritionists and diabetes nurse educators. Patients also gain access to NYU Winthrop’s Diabetes Education Center which is accredited by the American Diabetes Association, to offer self-management through education and counseling programs for adults and children with diabetes.
Obesity is an increasingly common chronic condition treated at the Center for the Advanced Care of Chronic Conditions. Through the Center, patients who are struggling with obesity receive a wide range of resources to lose weight and prevent or address the life-threatening complications of the condition. This includes medically-supervised weight loss and health enhancement programs conducted by a multidisciplinary team of weight experts, including endocrinologists, gastroenterologists, pulmonologists, cardiologists psychiatrists and bariatric surgeons, as well as nurses trained in the safe and specialized care of overweight or obese patients.
Multiple Sclerosis (MS)
Through the Center, patients with MS are given a variety of treatment options, assistance with medication management and education concerning how to best manage this chronic condition. This includes access to experts at NYU Winthrop’s Multiple Sclerosis Treatment Program who develop comprehensive and individualized plans to managing the condition and its long-term effects. Patients receive personalized medication and the latest treatment options, including medication and infusion therapies, new and emerging treatments and clinical research trials. Additional support includes physical therapy, psychological support and education.
Rheumatoid Arthritis (RA)
At the Center, patients with severe joint pain receive a comprehensive evaluation to determine the root cause of their pain and the best treatment options. If diagnosed with rheumatoid arthritis, patients will be referred to NYU Winthrop’s Arthritis and Rheumatic Disease Program, which offers a comprehensive approach to halting or slowing down the progression of this painful, chronic condition. Patients also receive a personalized treatment care plan to reduce pain and inflammation while limiting damage to the joints. Advanced treatment options available through the Center include clinical trials, the latest medications, biologic therapies, physical and occupational therapies and access to emerging research aimed at developing new agents to treat RA.
